About the Author

Head of the Department of Pediatric Ophthalmology and the Director of the Center for Genetic Eye Diseases at The Cleveland Clinic Cole Eye Institute. He is Professor of Ophthalmology at the Cleveland Clinic Lerner College of Medicine of Case University and Director of the Ophthalmology Residency Program at the Cleveland Clinic Foundation. He also serves as Vice-Chairman for Education at the Cole Eye Institute and Chairman of Graduate Medical Education at the Cleveland Clinic. He is Past President of The International Society for Genetic Eye Diseases and Retinoblastoma and its current executive vice-president. He has been the Editor-In-Chief of Ophthalmic Genetics sine 1992. He has authored more than 300 scientific articles and book chapters.

Great book, these cases are well written and for ...

Great book, these cases are well written and for some reason, it's easy to remember rare pathology when it's presented in a case based format. What I like the most in this booklet is references at the end....that really helped me to go further into the details of each case presented here.

As general neurologist, how many of you will diagnose and treat neuro sarcoidosis with infliximab. .....not a lot. Give it a try....you will be more confident treating some rare disorders.

Review of Neuroimmunology Pocket Book

As a first year neurology resident, i was looking for a book, that i can read for my two weeks Multiple Sclerosis rotation. As a big fan, of what i do now series, i thought i might give it a try. Book perfectly fits in your lab coat and it covers the basics of neuroimmunology very well. What i did was, i read the book, a week before my rotation, and this puts you in a great place to answers basic questions that your preceptor might ask in your rotation. Then during my rotation, i read the multiple sclerosis continuum to supplement it. Since this book was in my labcoat at all times, i could reference it, right after seeing the patients. Recommend to any one doing a MS/Neuromuscular rotation and even great for medical residents, that can use as a stepping stone to the world of fascinating neuroimmunology.

Useful Resource

I purchased this book to supplement my health psychology knowledge. I found it to be of immediate use and written in a manner that was easy to comprehend and would be appropriate for graduate psychology students. This is the best health psychology book I have found for use as a clinical resource. If you do consulting in a primary care setting, this book is highly useful. Even if you do not work in a primary care setting, this book can increase awareness of biological influences on mental health.

Richard B. Silverman is professor has published over 200 articles in the areas of synthesis, bioorganic chemistry, and medicinal chemistry, has been awarded 21 patents, and has invented a drug that is under consideration for commercialization.

About the Author
Professor Richard B. Silverman received his B.S. degree in chemistry from The Pennsylvania State University in 1968 and his Ph.D. degree in organic chemistry from Harvard University in 1974 (with time off for a two-year military obligation from 1969-1971). After two years as a NIH postdoctoral fellow in the laboratory of the late Professor Robert Abeles in the Graduate Department of Biochemistry at Brandeis University, he joined the chemistry faculty at Northwestern University. In 1986, he became Professor of Chemistry and Professor of Biochemistry, Molecular Biology, and Cell Biology. In 2001, he became the Charles Deering McCormick Professor of Teaching Excellence for three years, and since 2004 he has been the John Evans Professor of Chemistry. His research can be summarized as investigations of the molecular mechanisms of action, rational design, and syntheses of potential medicinal agents acting on enzymes and receptors.

His awards include DuPont Young Faculty Fellow (1976), Alfred P. Sloan Research Fellow (1981-1985), NIH Research Career Development Award (1982-1987), Fellow of the American Institute of Chemists (1985), Fellow of the American Association for the Advancement of Science (1990), Arthur C. Cope Senior Scholar Award of the American Chemical Society (2003), Alumni Fellow Award from Pennsylvania State University (2008), Medicinal Chemistry Hall of Fame of the American Chemical Society (2009), the Perkin Medal from the Society of Chemical Industry (2009), the Hall of Fame of Central High School of Philadelphia (2011), the E.B. Hershberg Award for Important Discoveries in Medicinally Active Substances from the American Chemical Society (2011), Fellow of the American Chemical Society (2011), Sato Memorial International Award of the Pharmaceutical Society of Japan (2012), Roland T. Lakey Award of Wayne State University (2013), BMS-Edward E. Smissman Award of the American Chemical Society (2013), the Centenary Prize of the Royal Society of Chemistry (2013), and the Excellence in Medicinal Chemistry Prize of the Israel Chemical Society (2014).

Professor Silverman has published over 320 research and review articles, holds 49 domestic and foreign patents, and has written four books (The Organic Chemistry of Drug Design and Drug Action is translated into German and Chinese). He is the inventor of LyricaTM, a drug marketed by Pfizer for epilepsy, neuropathic pain, fibromyalgia, and spinal cord injury pain; currently, he has another CNS drug in clinical trials.

Outstanding...

This is probably the best book of its kind in the vast and multidisciplinary field of drug design and action. Richard Silverman is an acclaimed chemistry professor at Northwestern University, and in this book he provides the most comprehensive and lucid exposition of the chemical basis of drug action that I have seen. No matter how complicated biological systems and their interactions are, at the basic level it's all chemistry, and mostly organic chemistry. Thus, understand the chemistry and you will go a long way in understanding drug action. An organic chemist will find this book extremely illuminating in a way that would not have been made clear through his or her traditional education.

The book opens with a short but clear discussion of drug discovery, combinatorial chemistry, patents and clinical trials. Then it leads the reader through a variety of enzyme catalyzed reactions, with examples of major drugs studded all the way. Silverman shows us how nature is the master organic chemist, employing the familiar reactions of chemistry in an unsurpassed manner. The chapter on enzyme inhibition is long and detailed and it's breathtaking. This is a key chapter because many major drugs act by inhibiting enzymes. The examples which Silverman chooses to illustrate are both important (including many bestselling anticancer, antiviral, and antibiotic drugs to name a few), as well as very interesting. The last parts of the book deal with DNA binding drugs and with pathways of drug metabolism and excretion. Along all the way, the emphasis is on the chemical reactions that drugs undergo, which are after all the key to understanding their properties. The lucidity of the book is such that a beginning graduate or advanced undergraduate well-versed with the basic principles of general and organic chemistry can easily understand all the contents.

I would say that this book is so interesting and illuminating that it's one of the very few academic books which you can enjoyably read cover to cover. In fact it even makes great bedside reading! Truly a must-have book for chemists and biologists of every kind interested in how drugs work.

Fantastic Book for chemists and medicinal chemists

Some books of this nature tend to focus on the synthesis of drug molecules and less about how they work and why. This book is just the opposite and walks the reader through a logical progression of drug discovery, design and target specificity. This book makes a great introduction to Goodman & Gilman's The Pharmacological Basis of Therapeutics which could be considered the bible of the subject.

Excellent read compared to Foye

We use Foye for Pharmacy students and this text distinguishes itself by being an easier read and having more illustrative examples. That said, it won't replace the drug-class focused chapters in Foye, but this is a much better textbook for fundamentals of medicinal chemistry.

Lost in translation

The back cover of this book describes it as "Written in accessible language..." The dedication of the book includes this: "This book offers hope, discovery, and translation. It is hoped that the partnership between the stroke survivor and informed therapist will ensure that this journey does not end, but rather keeps open the "window of opportunity".

Are there any therapists or survivors interested in the methodological distinction concerning source modeling between magnetoencephalography and electroencephalography?

The word accessible shouldn't be anywhere near this book. These authors should heed the words of the great Kurt Vonnegut, "Simplicity of language is not only reputable, but perhaps even sacred."

Scientists of the world: accessibility is quantifiable. You can predict the level of education needed to read prose. This is known as "readability."

I randomly picked out one sentence by literally leafing through pages and letting my finger drop. It landed in the middle of page 79, on this sentence: "Apparently, the residual flow associated with an intracranial occlusion before systemic thrombolysis predicts the likelihood of complete recanalization and long-term neurological outcome."

Another problem: This book presents a perspective about tools for rehabilitation after stroke that is negative, unproven and ultimately, inaccurate. Let's say you were clinician, working with stroke survivors, and you wanted to see what would help stroke survivors get better. What's the most widely used treatment options for stroke recovery in this country, and probably worldwide? Neurodevelopmental treatment. Weirdly, the authors never even mention this highly unproven, widely used option. Instead everything else is trashed. And they do it using "Insights from neuroscience and imaging."

-Repetitive task specific training: very few studies have examined the central nervous system effects, and what has been studied is limited by small sample sizes.
-Constraint induced movement therapy: few studies, small sample sizes
-Mental practice: few studies, small sample sizes
sensory recovery after stroke: relatively few studies...
-Robot assisted training: few studies, no evidence of neural effects
-VR, the same.

If it is to be believed that there is a paucity of evidence for neural substrates needed to prove efficacy, maybe the contributors should work on that rather than writing a book about how the work hasn't been done. The problem seems obvious: the rehabilitation research has been done to show kinematics and functional changes, but the neuroimaging has not yet caught up. This is a pretty large frustration among rehabilitation researchers. Most notably, Edward Taub has pointed this out. When he showed functional changes, nobody cared. It was only when brain scanning showed central changes that everyone went "A-ha!" It is not particularly fair to condemn treatment options that has proven kinematic benefit because central changes have not yet been proven.

Perhaps this book should have been written 10 years from now, once the neuroscience and imaging had done its due diligence.

The reason I'm giving it four stars is because this book does provide a concentrated source of information about neuroimaging and stroke.

Predicting and Controlling Brain Activity

Niels Bohr has been quoted as saying "the hardest thing to do in science is to predict, especially the future!" This book is concerned, in large part, with this problem, in its application to controlling the dynamics of the brain. The problem involves collecting observations of brain activity, filtering out the effects of noise and measurement errors, and controlling brain dynamics. In many ways this set of problems originated in Gauss's least squares method of minimizing the effects of measurement errors, which is over 200 years old. But it was during the period between World Wars 1 and 2, that the current methods for dealing with noisy data in the exercise of predictive control were really started by Norbert Wiener, and by Andrey Kolmogorov in 1941. Wiener's theory considered data in the form of continuously changing data represented by stationary random processes with Gaussian statistics, and Kolmogorov studied very similar processes generated by data sampled at discrete times. Essentially Wiener and Kolmogorov developed filters to remove the effects of noise and measurement errors from the data, so the theory is often called the Wiener-Kolmogorov filtering theory.

The problem with both theories was that they dealt only with linear, stationary and Gaussian processes. It took another 20 years before Rudy Kálmán (1960) introduced a filter theory to deal with noisy dynamical systems, and another 40 years or so before the Kalman filter techniques were used to estimate the parameters of dynamics generated by various equations describing neural activity. Schiff's book is the first comprehensive account of this development. Schiff is perhaps uniquely qualified to write this account. He is both a practicing neurosurgeon and computational neuroscientist, and a pioneer in the application of control techniques to problems such as chaos control.

The early chapters provide a brisk introduction to least squares minimization and its connection with Bayes Rule, and thence to iterative methods for data assimilation, i.e., the processes that incorporate measurements into various models of neural activity, in particular to the Kalman filter approach for discrete data, and the Kalman-Bucy filter (1961) for continuous data. The first neural examples considered are the Hodgkin-Huxley equations (1952) that model the ionic current flows that trigger current pulses in neurons, and various simplified models such as the Fitzhugh-Nagumo equations (1961). Schiff shows that such simplified models often lead to effective controls of neuronal activity. In keeping with this, the next chapter deals with a population model of large-scale neural activity, the Wilson-Cowan equations (1972, 1973). These are essentially a spatio-temporal extension of Fitzhugh-Nagumo like equations, and Schiff shows how a Kalman filter approach can again be used in an efficient way to control their dynamics. The final two chapters of this sequence deal with the construction of ab initio models and filters based directly on data assimilation, and with the inadequacies of all such models. Chapter 7, the first of these two chapters, studies the utility of such techniques as Principal Components Analysis, and Singular Value Decomposition and other techniques, as ways of abstracting from data sets, a set of uncorrelated linear combinations of parts of the data set, that carry the essential information in the set. The first few such components can then be used in a Kalman filter approach. Examples are described of applications to Image Analysis, both static and dynamic, and to the analysis of Spatiotemporal Brain Activity.

The final chapters, 9 through13 are perhaps the most interesting. Chapter 9 covers such topics as Apostolos Georgopoulos' discoveries in 1982-1986, that the direction of a limb movement is uniquely predicted by the activity of a (small) population of neurons in the monkey motor cortex. Essentially each neuron in the population is tuned to respond maximally for a preferred direction of limb movement, and the vector sum of the population activity drives the intended movement. [There is a corresponding effect in the visual cortex: the vector sum of the population activity of a (small) population of neurons in the visual cortex accurately represents the local orientation of the edge of an object in the visual field, as was demonstrated by David Hubel and Torsten Wiesel in 1959-1974.]
These findings were seminal for the development of brain-machine interfaces in the form, for example, of implanted arrays of microelectrodes. The resulting deluge of data required much assimilation via Kalman filters. Chapter 10 provides a very interesting introduction to Parkinson's Disease, and the models thereof developed by David Terman and colleagues (2002-2004). These models are simplified Hodgkin-Huxley-like models, and provide a first attempt to model Parkinson's Disease and to provide insight into the efficacy or otherwise of deep brain stimulation. Chapters 11-12 provide a brief look at the use of electric fields to stimulate the brain, and at recent attempts to understand and control Epileptic Seizures. The final chapter, 13, is more speculative, but raises the possibility that brain themselves implement Kalman filters.

In summary, I found this book extremely interesting and well written. I have only some minor caveats. There is almost nothing about Wiener-Kolmogorov filters. In Chapter 11 the method of reducing the resistive tree structure of a neural dendrite to an equivalent cylinder is introduced with no citation of Wilfrid Rall's introduction of this method (based on impedance matching) in 1957-1960. Apart from these, this book is a gold mine for anyone interested in how to model brain activity, and how to control it.

Interdisciplinary Gem

In terms of what this book is about, I have not much to say beyond what a previous reviewer said. What I want to emphasize is the extraordinary fact that all these chapters are offered by the same author. He has obviously spent many years of his life to learn and practise enough engineering, mathematics, statistics, physics, philosophy - beyond his primary profession as a neurosurgeon trained in physiology - to meaningfully bring together what is needed to advance his field beyond this point.

Why should you read this book - beyond admiring the last polymath?

Case 1: You plan to attend Prof. Schiff's Course - well then it's obvious

Case 2: You plan to teach a course in a related field - then you can take this text as a blueprint for your preparation

Case 3: You plan to do work in neural control engineering - then you will find your learning agenda set in this book. You will probably master one or two of the branches covered, but you need to develop some intuition on the others as well

Case 4: You are just curious what is going on in this area - that's how I came to read it. Then you will understand what it is like to go beyond trial-and-error in a field where full understanding of the inner workings of the system is way out of reach.

The book is a very entertaining demonstration that intuition is of utmost importance in interdisciplinary work - and that intuition comes as the result of hard work.

What I personally enjoyed is that where other authors offer bold statements and hyperbole, Steven Schiff shows courage and hope. That is a major difference.

Hope and courage is needed in particular for the last chapter "Assimilating Minds". The author is well aware that anything achieved so far in neural control engineering looks like mere stamp collecting if you had set out to understand the mind - as many did. But why not take the finest stamps from distant countries as inspiration and encouragement to eventually go to these places yourself!

The volume is staged as a serious text in a specialized field. So color illustrations are offered not for glamour but where it helps to understand a figure. For a high quality hardcover edition of a cutting edge text, I find the book reasonably priced.

A Research Monograph on Neuronal Models and Related Filtering Problems

This is a very nice interdisciplinary book. It deals with neuronal models and a number of issues related to filtering and model uncertainty. This book will prove helpful for those who need to grasp knowledge about neurons, models and data from both. There is much information on historical development of the field which is an extra benefit. The author succeeds in making a strong point for Kalman filtering (in one of its nonlinear versions, the UKF) as a tool for handling, model, data and uncertainty. The book, which is very well written, reads more like a research monograph than a textbook and the bias is clearly towards neural issues albeit the author's efforts to relate the material to control engineering. Given the "plant" considered by the author, it is fair to say that his efforts have been quite successful.
